In my minds eye and although, I can handle blood and guts stuff...A hairy pus filled infection (as it has been described) on someones backside is not something I would relish in!

As I said in my reply PM, I'm not the BelllyDoc. It looks like some sort of exema or skin irritation that may have become chronic if you've had it 10 years. It looks like bad poison ivy that you've irritated. If it was a field medical situation, I'd put benedryl on it -- it doesn't look at all "surgical". Just very topical on the skin.
