I went across the country (from Atlanta to Spokane) on a month-long trip and found that running regular (85/87) was more expensive than running primium (91/92/93) the savings was "per tank cost" ; a savings of about $3.00 - $4.00 per tank. The reason was the increased mileage running primum verses the reduced mileage of running regular.
Our trip took us from 900 ft. elevation to 10,000 ft. elevation. I did notice improved performance in the colder temperatures.
The headwinds of Kansas really took it's toll on mileage (12/13 MPG) What a long state!
I am running primium still as of this date.
I'm with others... When it's recommended by the mfg - I do it!

I am at altitude, if that makes a difference (yes, I am a newbie) and when gas went up to $3.50 a gallon for premium, I started putting in 89 octane. After about five tanks, I did notice timing hitches. I started putting 91 back in and the odd hitches and such have almost stopped.

Stay on the trail or I'll have to shame you into behaving.
It depends, if I am going HIGH altitude wheeling, I usually tend to put in low octane especially since there is not a lot of air up there anyway. That and with the 91 already in the tank the higher octane balances out the low octane.
In my other vehicle I use a gallon of 76 Cool Blue race fuel to regular 91 and my motor doesnt ping. If I run pure 91 it pings like pouring a box of Kix cereal into a glass bowl.
This is where I get that idea from in mixing medium/low octane with CA version of "high octane".
