I used to haul gas for a living. It is not possible to use midgrade and have consistent octane ratings because it is actually a mix of regular and premium at the tank farm where fuel tankers fill up for delivery to the gas station. The fill stations at a tank farm have minimum increments of 500 gallons that can be loaded by the driver. If the customer (gas station) orders 1000 gallons then the driver can mix 500 regular 87 and 500 premium 93 and get supposedly 91 midgrade. But what happens when customer orders 1200 gallons? 500 regular and 700 premium would be what you hope you are getting, but it can also be 700 regular and 500 premium whatever octane (less than 91 though) that would be. If you are the tanker company and trying to maximize profits, then driver will probably load 500 premium/700 regular, this is true for every brand in California, except Chevron and Shell/Texaco, they have quality control that ensures minimum octane ratings for midgrade. The cheapo stations all have alcohol in their fuel, since alcohol will absorb water right from the atmosphere (just like brake fluid) all this gas has water in it, ever seen a car with water running out of the tailpipe? All that water just went through your exhaust cats and mufflers! I think that if gas price from a cheaper source is not worth the wter damage to the vehicle is not worth it, like 10 cents per gallon only means a full tank in an FJ tank would mean $1.90 for a whole tank fill to avoid damage to hundreds of dollars worth of exhaust gear.

The 4.0L is the same Tacoma engine that ran off of 87 octane, Toyota just made it work with 91 so it would pass recent smog standards, it still works just fine with 87 octane. The 4.0L is not a high performance enegine that needs 91, but if you smog it, fill it with the 91 a few times so you pass smog.

Again, the 91 is only so it passes emissions standards to be rated in a spceific category which I can not recall the name of at the moment, OEG or something like that.

I drove mine from Portland, OR to Louisville, KY, then to Oklahoma City, then back to Portland via Arizona then up through Utah. 5500 miles total. I ran 91 in it all the way out to Louisville and only got 16.2 mpg. The rest of the way I ran 93 and got 19.9. Better mpg and it just felt better too, especially in spots like going through the Blue mountains in eastern Oregon!

the past 2 fill ups I decided to give 93 octane a try (I had been using regular 87 since day 1 with this truck) and low and behold, Im getting 2.5 mpg better now which makes running super cheaper than regular, not to mention my range has extended quite a bit.

we mixed up some 100 octain av gas with some premeum and wow my engine came apart at 88mph at 5400 rpms..........................

I have ran 87 since my first fill up, however, I do fill up with 91-93 every 4th tank or so. I have had no change in performance with the cheap stuff and gas mileage hits at around 16-19 miles per gallon. I drive through a lot of construction areas so I can't drive that fast anyway. But took it to Mich. U.P. and averaged around 18 mpg there and back. Loved the dunes up there, you got do the dunes.
With that being said, I think I may start to run 91-93 (whichever I can get from Speedway) as a rule through the winter and on trips. I have not took her through any hilly areas yet so can't say how fuel affects performance in that situation.
