We have certain runs that I have started requiring it on. Baja, the Arctic Ocean, etc. Bigger runs, I want everyone on 2M. When I can't I usually ask the tail gunner to at least have 2M. When you get a bigger group spread out around terrain, you can't beat the reach of 2M (or 440).
I have a handheld cb and a hard mounted 45w 2m radio. The cb has not been out for probably 2 years now. Most clubs and even informally organized runs here have transitioned to 2m. Honestly, the little FRS/ GMRS radios work as well or better than a cb IMO. I refuse to waste any more time or $ on a cb.

I just got sick of the constant fussing necessary with a hard mount cb. Inevitably someone's radio needs tweaking and even when they "work" range is very limited.

As far as a 2m license, its really just a function of devoting a few hours to studying and taking the test. No big deal.

Re: CB radio

Instead of cutting into the factory boot, I recommend running the coax straight down the door forming a "drip ring" so water goes away from the hole.
Then the cable runs up into the door, through a grommet, into the cargo door then through the factory rubber boot (without cutting into it) then into the FJ.

I recommend doing your own research but I would recommend these 2 items as alternatives to the Firestik brand. I use these:

Wilson 18' Coax: http://www.amazon.com/gp/product/B00...iglink20486-20

Wilson 4' Antenna: http://www.amazon.com/gp/product/B00...iglink20486-20

Oh, and here's a photo from my install...
