No grounding needed on the mini add-a-circuit. Sounds like a bad switch maybe? Try taking your 86 wire and applying +12V and see if the relay fires. If the relay fires, you can work back through your switch (check continuity through the switch terminals) and then your mini add-a-circuit tap.

Good idea, but I checked it and I do have power to the switch. Not sure of how many volts - my meter is down - just using a test light. Showld be 12V though if I am getting power.
Check all the connections on the relay. Power (verify at the relay with test light) as well as GND (using the test light in the opposite sense, applying 12V to it and grounding it on the 85 terminal) and the feed.

Sounds like you've eliminated your switch, I wouldn't put it past a bad relay or bad connection somewhere.

You said you've got no power to the 30? That means you've got a supply issue. 30 connects to 87 when 85/86 are a circuit. Check your supply line going into 30 from the battery and make sure it's good.
